How to Transfer to Our Rehabilitation Hospital

The Transfer Process
To begin, you or your physician can contact our admissions office to request a pre-admission screening. One of our experienced rehabilitation liaisons will complete the evaluation with input from you, your doctor, and your family to determine eligibility for acute inpatient rehabilitation.
Once approved, we’ll coordinate your rehabilitation transfer to our facility—often within the same day. Our goal is to get you started on your rehabilitation program as quickly and safely as possible so your recovery can begin without delay.
