Accel Rehab Hospital of Plano Home Admission Program assists persons who have had a recent, significant loss of function due to a known condition or well-defined diagnosis, but do not require a acute care hospital admission.
Who Qualifies:
- Stroke patients, Degenerative/progressive neurological disease, i.e., MS, Parkinson’s or other types but are not limited to.
- Frequent returns to the doctor’s offiice for the same diagnosis or symptom management
- Are experiencing frequent falls and/or difficulty managing activities of daily living
- Would benefit from inpatient therapy/medication management
Once a referral is made a Clinical Liaison from Accel will conduct a comprehensive evaluation to determine if the patient meets criteria for acute rehabilitation and will follow up with the referring physician.
