Patient Outcomes from ACUTE Rehab

At Acute Rehabilitation Hospital of Plano, our goal is to provide medically and procedurally excellent rehabilitative care. Our team of expert, experienced physicians, nurses, therapists and support staff all have one common goal—to provide quality, efficient, safe and cost-effective care to restore patients to an independent, rewarding lifestyle in a thorough, expedient manner. We want our patients to regain maximum independence and strength by way of a rehabilitative program designed personally for them.

We use the Inpatient Rehabilitation Facility (IRF) Quality Reporting Program (QRP) to measure our patients’ progress and to show improvement. Developed by Center for Medicare & Medicaid Services (CMS), the IRF-QRP is intended to standardize post-acute care data to improve outcomes through shared decision-making, care coordination and enhanced discharge planning. While a patient at Acute, we use this data to evaluate and track a patient’s functional status upon admission and discharge, assessing rehabilitation effectiveness and benchmarking ourselves against regional and national performance data.
